1. Download and print out our medical application form and our personal application form.
2. Submit your applications by mail or by fax at 416-923-1343.
3. We review the applications of each resident to assess whether Fellowship Towers will make the ideal home.
4. We will then invite you for a meeting, where we can discuss whether Fellowship Towers is the right home for you.
